D2L 2025 Assignments

Please read the attachment and submit only the last 2 pages signed with date. (Note: A hard copy of the required signed pages will be placed in your yellow folders.)

If you have not submitted these forms in the past or your withholdings have changed, then be sure to submit a printed version (or turn them in digitally via Google Classroom).

NOTE: Payroll distribution will take place on Thursday, June 26th. I'll need these forms by latest, June 19th.

I have not processed payroll for you in the past for D2L.

You want to update your withholdings.
* Due by Thursday, June 20th to allow payroll to be processed on time. (Checks will be distributed on June 27th.)
