2. VERIFICATION OF RESIDENCY
a. Property tax, lease agreement or mortgage statement
b. Recent utility bill (gas, water, electric)
c. CA Driver's License or CA Identification with the SAME address shown on documents (a-b)
3. STUDENT DOCUMENTS
a. Original birth certificate
b. Immunization record
c. Recent report card
